A Sudden Turn After Birth
Born at full term through a normal vaginal delivery, the baby showed promising signs at birth, crying
soon after delivery. However, within a few hours, the baby began to show signs of respiratory distress
and was immediately shifted to the Neonatal Intensive Care Unit (NICU).
A Race Against Time
On the very first day, the baby developed severe abdominal swelling. An urgent X-ray revealed
Pneumoperitoneum, indicating a life-threatening perforation in the intestines. An
emergency surgery (laparotomy) was performed, and doctors discovered a mid-ileal perforation with
massive fecal peritonitis. A temporary ileostomy (a surgical opening in the intestine) was created to
save the baby’s life.
Managing a High-Risk Condition
After the surgery, the baby was started on breast milk. However, the stoma output was abnormally high,
leading to weight loss from the birth weight of 2.5 kg. To stabilize the baby, breast milk was
temporarily stopped, and Total Parenteral Nutrition (TPN) was initiated.
With dedicated care, a tailored nutrition plan was introduced. This included:
- Cholestyramine
- Loperamide
- Probiotics
- Simyl MCT (a specialized formula with hydrolyzed protein)
The baby’s electrolytes, fluids, and nutrition were closely monitored and adjusted daily. Despite facing
extrauterine growth retardation, the baby’s head circumference and length stayed on track, thanks to
timely intervention and expert monitoring.
A Milestone Achieved
At two months and ten days, with a body weight of 2.9 kg, the baby underwent a successful stoma closure
surgery. Feeding was resumed on day 5 post-surgery. Even when the baby faced a central line-related
infection (CLABSI), our medical team responded swiftly with the right treatment.
Soon after, the baby began gaining weight, hitting an emotional milestone that brought smiles to the
entire NICU: the first Social Smile.
